CLIENT ELIGIBILITY

The Community Care Program is an entitlement program. All elderly people who are eligible for the service can receive it. There is no waiting list. To receive homemaker services through the Community Care Program, Client Should fulfill the listed Conditions.

» Age 60 or 60+
» A resident of Illinois
» A US Citizen or Legal Permanent Resident
» Have Difficulty with activities of Daily Living (eg. Eating, Dressing, Bathing) and
   instrumental activities of daily living (e. Cooking, Shopping, Performing Laundry)
» Provide Proof of Income
» Pay for a portion of the care provided, unless the client has a very low income
» Not have assets (Such as bank Accounts) of over $10,000.00. The house and some
   other assets are not included.

HOW TO BE A MAFS CLIENT

Any person who fulfills the above mentioned Criteria, can Apply for this program by visiting us at MAFS and filling out the referral form or can fill the online Application and we will mail your Application to the Illinois Department on Aging and a Case Manager will visit you for Assessment.

Client must have an assessment by a Case Manager to establish the need for Service, the client's eligibility for service, and the plan for the amount and kind of service to be provided. Case Managers usually conducts the assessment in the client's home. If necessary, the Case Manager must find an interpreter when conducting Client's assessment. A family member or a Staff Member of Metropolitan Asian Family Services should not be the interpreter, as this is considered to be a conflict of interest by the Illinois Department on Aging.

Clients are referred to MAFS from the CMU. Service for the Client will Start within 15 days if the Client is referred from a Case Manager.

HIRING PROCESS

It is the Policy of MAFS to recruit and hire Qualified workers who enjoy working with the elderly. MAFS recruits through:

  • Newspaper Advertisements
  • Flyers Placed in ethnic Business and in Temples, Mosques & Churches frequently by the target ethnic group.
  • Word of mouth

 

Individuals who apply for a position as a Home maker must fill out an Application for Employment at site. The Application for Employment gathers information from the candidate including:

Name, Address, Telephone
Social Security Number
Education History
Former Employee
General Information
References

If the information on the Application for Employment indicates a candidate that may be qualified for a Homemaker position, the Homemaker Supervisor conducts an initial interview with the candidate. This interview reviews the information provided on the Application for Employment, and asks further questions of the candidate, including reasons that the candidate has for wanting the position and languages spoken. In the initial interview, the Home maker Supervisor shows a copy of the job description for the Home maker position to the applicant, and verifies that the candidate is willing and able to perform the enumerated tasks. The Homemaker Supervisor also reviews salary and benefit information with the candidate to ensure that these are acceptable to the candidate.

If a Candidate successfully completes the initial interview, the Executive Director conducts a second interview with the candidate. When the Executive Director accepts a candidate for employment, the candidate is asked to sign a waiver for Universal Industries to conduct a background check on the individual. Background checks are conducted through ASI. A Candidate whose background check reveals a prior conviction for any crime listed in the Health Care Worker Background Check Act is not offered employment.

Once a candidate has been offered and accepts a Homemaker position, the candidate receive the Statement of Employment. The Candidate signs this statement to show his/her acceptance of the starting date and salary. The candidate provides Documents to MAFS and MAFS reserves the right to verify the documents. Homemaker Supervisor makes a copy of the documents for the employee's file. The candidate must also complete a medical review, at the candidate's own cost, including a TB Skin Test.

Before being placed with a client, the Homemaker must complete pre-service training (See Homemaker Training). When the Homemaker is placed with a client, the Homemaker Supervisor reviews the Plan of Care for that client with the Homemaker and ensures that the Homemaker clearly understands the duties that he/she is and is not authorized to perform for the client.
